Facility Evaluation Report

Licensing Program Analysts (LPAs) Janira Arreola and Imaculada Vasquez, conducted an unannounced visit to the facility in order to conduct a case management visit. LPAs spoke to Licensee, Mary Martin over the phone during the visit. LPAs met with Staff, Marlene Martin, who was informed of the purpose of the visit. LPAs conducted interviews, records review and walk through during the time of the visit.

During the time of the visit there were (2) clients and (1) staff present. LPAs observed client rooms, food supply, utilities, and criminal record clearance for staff. LPAs observed no immediate health or safety issues during the time of the visit. An exit interview was conducted where this report was reviewed with the staff. A copy of this report was provided.
