Facility Evaluation Report

On June 11, 2026, Licensing Program Analyst (LPA), Jarred Torres, arrived at the facility unannounced to conduct an annual inspection. LPA knocked on the door and rang the doorbell three times, waiting a few minutes each time. There was no answer. LPA called the listed Administrator, Mary Martin, at the last known phone numbers 951-924-0656 and 909-948-3773; however, there was no answer. So, LPA could not perform the annual inspection during this visit.

LPA observed the front yard in disarray as there were large patches of dead grass, fallen branches from the trees, and scattered dead leaves and flowers on the ground. There were no cars parked in the driveway and no cars parked on the street outside the home.

Based on entries in the Field Automated System from June 4, 2026, the previous clients have been moved out of the facility.

Based on LPA's observations, there are no clients or staff present, and the house appears vacant.
