Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced visit to the facility in order to conduct a case management visit. LPAs met with Staff, Bonifacio Gene Carlos, who was informed of the purpose of the visit. LPAs conducted interviews and walk through during the time of the visit.

During the time of the visit there were (2) clients and (1) staff present.

LPAs observed food supply in the facility meet the 7-day supply of non perishable foods and met 2-day supply of perishable foods. LPA observed working utilities and both clients in care. LPA observed no immediate health or safety issues.

No deficiencies were cited during the time of the visit. A technical note was documented for the licensee to attend to the front yard over grown weeds.

An exit interview was conducted where this report was reviewed with the staff. A copy of this report was provided.
