Facility Evaluation Report

Licensing Program Analysts (LPAs) Janira Arreola and Seo Jeon, conducted an unannounced visit to the facility in order to conduct a case management visit. LPAs met with Staff, Bonifacio Gene Carlos, who was informed of the purpose of the visit. LPAs conducted interviews and walk through during the time of the visit.

During the time of the visit there were (2) clients and (1) staff present. LPAs observed no immediate health or safety issues during the time of the visit.

LPAs observed food supply in the facility meet the 7-day supply of non perishable foods and met 2-day supply of perishable foods. Therefore, the deficiencies were cleared from the previous visit. An exit interview was conducted where this report was reviewed with the staff. A copy of this report was provided.
