Facility Evaluation Report

Licensing Program Analysts (LPA) Abdoulaye Zerbo, conducted an unannounced visit to the facility for a health and safety check. LPA met with Caregiver, Maribel Aguilar who was informed of the purpose of the visit.The Licensee was available over phone at the time of the visit and LPA inform them of the reason of the visit.

LPA conducted a walk through of the facility and observed 4 canned food with the expiration date between 2023 and 2024. Caregiver discarded the items in the presence of LPA, so no citations were issued. During the walk through, LPA observed three dead cockroaches in the first closet in the hallway glued to a trap at the bottom of the closet. The trap was discarded by Caregiver Maribel. Citation will be issued. Additionally, during the exterior inspection of the house, the LPA did not observe any sitting area for the clients in care. Citation will be issued.

An exit interview was conducted and a copy of this report, 809-D and the appeal rights was reviewed and a copy was provided to Caregiver Maribel Aguilar
