Facility Evaluation Report
Licensing Program Analysts (LPAs) Janira Arreola and Venus Mixon conducted an unannounced visit to the facility for a required annual inspection. LPA was met with Administrator, Joshua Schock who were informed of the purpose of the visit. There were (2) staff and no clients present at the time of the visit.
Facility is a two story home with (4) client bedrooms and (2) client bathrooms on the first floor. The facility has a pool with the required locked inaccessible fence surrounding it. No firearms are being stored at the facility, The facility is approved for (4) ambulatory and (2) non ambulatory clients. LPA observed the physical plant floors doors and windows, fixtures and furniture were in good repair and were present. There are extra linens and hygiene items, for clients and cleaning supplies to ensure the facility is sanitary. Facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. Facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods. The smoke alarms and carbon monoxide detectors were operational and the hot water was 114F. The outdoor area has shaded area for clients and had a clear exit path.
(1) Client files and (1) staff files were reviewed which possessed all required documents. All staff present are fingerprint cleared. The administrator has a current administrator's certificate. The client personal and incident funds were reviews, the ledgers and money were accounted for.
