Facility Evaluation Report
On June 06, 2025, Licensing Program Analyst (LPA), Venus Mixson made an unannounced visit for the purpose of the facility's closure and met with Administrator/Licensee, Marquita Berry.
The Licensee is initiating this closure on 06/04/2025 with effective date of closure 06/06/2025. LPA Mixson inspected the facility inside and out.
There were no observable regulation violations at the time of this visit.
LPA Mixson confirmed there were no residents present, and there are no personal belongings for any resident. Licensee stated the reason for closure is due to change of facility type. The Licensee informed LPA there were never any residents placed.
Licensee surrender a copy of the License (Effective Date: 01/12/2024), with a Facility Number 331881482. Total Capacity: 6
An exit interview was conducted with Administrator, Marquita Berry and a copy of this report was provided.
