Facility Evaluation Report
On 11/4/2025, Licensing Program Analysts(LPA) Valerie Flores conducted an unannounced 1-year required annual inspection . LPA Flores was greeted and granted entry by House Manager, Natali Cabrera and explained to Natali the purpose of the visit. During the time of visit, (4) four out of (4) four clients were attending day program. LPA Flores conducted a tour of the facility alongside Natalie and observed the following during today's visit:
The facility is a single-story structure which consist of (4) four client bedrooms, (2) two bathrooms, kitchen, living room, staff office, garage, and backyard area. The kitchen area is clean and free of litter. The facility maintained a (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. There is a locked container located under the kitchen sink that stores knives and other sharp instruments. LPA observed a locked storage closet that stored cleaning supplies and disinfectants. Client bedrooms were equipped with the required bedding, lighting, furniture, seating, and functional lighting. LPA observed emergency food and water located in the garage. There were activities like crafts and games located in the facility hallway to encourage client interaction. Facility maintained extra linen for client use and were observed to be in good repair. The medications are stored in a locked cabinet in the office and inaccessible to the clients. Smoke and carbon monoxide detectors were tested and were observed to be fully operable. Indoor/outdoor passageways were free from obstruction. LPA observed outdoor seating with sufficient shading. Per Administrator Juanita Sanchez, there are no firearms and/or ammunition on the premises.
LPA conducted of records review of the facility files. LPA observed that the facility conducted emergency drills monthly. LPA conducted a resident records review; Records included but not limited to current IPP's, admission agreements, preplacement appraisals, weight charts. safeguarded cash resources, and medical assessments. During today's visit, LPA did not observe any health and safety concerns.
An exit interview was conducted and a copy of this report was provided to Administrator Juanita Sanchez.
