The inspector’s account

On 02/25/2026, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted by Caregiver Leniejoy Rosales who was informed of the purpose of the visit. Licensees Genevieve Sengdala and Damon Dunclayan arrived to the facility and were also informed of the purpose of the visit. The facility has a fire clearance to serve four (4) ambulatory clients. During today's visit, LPA observed two (2) staff and four (4) clients to be present.

LPA toured the facility with Caregiver Rosales and observed the facility is made up of a one-story home with four (4) client bedrooms, two (2) bathrooms, a kitchen, dining area, living room, staff office, laundry room, and attached garage. Client bedrooms had the required bedding, furniture, and lighting. Bathrooms had toilet paper, soap, and paper towels readily available. No bodies of water were observed on the premises. Indoor and outdoor pathways were free of obstruction. The facility met Departmental requirements for a two-day supply of perishable foods and seven-day supply of non-perishable food items. Medications are secured in a locked kitchen cabinet. Licensee Duclayan tested one (1) of the smoke alarms/carbon monoxide detectors and LPA heard them to be operational. LPA also observed a charged fire extinguisher mounted near the kitchen last serviced on 01/17/2026. Staff present have a criminal record clearance and are associated with the facility. Client files reviewed had updated physician's reports and Individual Program Plans. LPA reviewed the Record of Client's/Resident's Safeguarded Cash Resources (LIC 405) for three (3) clients while Licensee Sengdala counted their physical monies and no discrepancies were discovered. LPA reviewed the facility's Disaster Drill Log Form noting the facility's last fire drill was conducted on 01/16/2026 and earthquake drill on 02/19/2026. Clients' personal rights, complaint procedures, emergency disaster plan and facility sketch are visibly posted near the front entrance. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Licensee Sengdala.
