Facility Evaluation Report

Licensing Program Analyst (LPA) Paola Guerrero arrived at the facility to conduct a case management visit. Upon arrival, LPA met with Facility Manager Charles Okoji. On 7/1/2026, the Licensee received an approved fire clearance from Lake Elsinore Fire Department for the approval of one (1) non-ambulatory client. Bedroom #1 was inspected and approved to have one (1) non-ambulatory client. Capacity of 4 will remain the same, totaling three (3) ambulatory and one (1) non-ambulatory. LPA will update the facility's file and issue a new license stating the change for Bedroom #1 being approved for one (1) non-ambulatory.

An exit interview was conducted where this report LIC809 was discussed and provided to Facility Manager Charles Okoji at the end of the visit.
