Facility Evaluation Report
On 05/15/2025 at 10:00 AM, Licensing Program Analyst (LPA) Raquel Hernandez conducted an announced visit to the facility for purpose of Pre-licensing evaluation. LPA met with Administrator/Applicant Telecia Royal. An initial application to operate an Adult Residential Facility (ARF) was submitted to the Central Applications Bureau (CAB) in August 2024 for a total capacity of four (4) Ambulatory. Fire clearance was granted on 03/04/2025. LPA Hernandez observed the following:
Facility was a two (2) story house with four (4) client bedrooms, one (1) staff area and three (3) bathrooms, living room, dining area and kitchen.

Each client bedrooms accommodate any ambulatory client. All client bedrooms were adequately furnished
with bed, chair, closet, appropriate linens, adequate lighting.
The three (3) client and (1) staff bathroom have a working toilet, wash basin, and shower with an adequate supply of toilet paper and soap. LPA Hernandez tested the water temperatures in the clients' bathrooms. Water temperature tested within regulation at 120 degrees Fahrenheit.
