Facility Evaluation Report
Licensing Program Analyst (LPA) Andrew Martinez conducted an unannounced visit to the facility to conduct a required comprehensive annual inspection. LPA met with and introduced themselves to facility staff member, Henry George- Direct Support Professional, who granted LPA entry into the facility. Administrator Muideen Salami was notified of LPAs presence and arrived at the facility shortly after. The facility is a (4i) Adult Residential Facility with a current census of (2) clients. The facility is a certified vendor for Inland Regional Center.
LPA, accompanied by Administrator Salami, conducted an overall inspection of the facility which included but was not limited to the following:
Operation/Physical Plant: The facility temperature ranged at a comfortable 75-77 degrees F. Indoor and outdoor passageways were kept free of obstruction. The facility has no swimming pools or similar bodies of water. The facility has sufficient indoor and outdoor activity space for client activities. Outdoor activity space is covered and enclosed with a self-latching gate. The facility is equipped with laundry equipment, smoke alarms and carbon monoxide detectors, and (2) fully charged fire extinguishers. The facility has sufficient bed linen and personal hygiene product supply for clients in care. Client bedrooms were equipped with beds, bed linen, nightstands, storage space and lighting. Client bathroom toilets, wash basins and showers were fully operational; however, LPA observed toilet seats and toilet seat covers needing replacement in (2) bathrooms. (1) type B deficiency cited. Hot water temperatures at taps delivering water ranged from 105-114 degrees F. The facility has the facility's license, house rules, Ombudsman poster, emergency telephone numbers, and Administrator's certificate posted in a common area. Medications are centrally stored and kept in a locked cabinet. LPA observed telephone service to be non-operational during inspection, to which Administrator Salami informed LPA that telephone service provider Spectrum would be sending out a technician to repair equipment and re-establish telephone service. (1) type B deficiency cited.
