This is an official report of an unannounced visit/investigation of a complaint received in our office on

02/09/2022 and conducted by Evaluator Jesse Gardner

COMPLAINT CONTROL NUMBER: 18-AS-20220209084608

Resident sustained an unwitnessed fall resulting in hospitalization.

Resident's request for assistance was not responded to in a timely manner.

On 02/16/22, at approximately 1030am, Licensing Program Analyst (LPA) Jesse Gardner made an unannounced visit to the facility to investigate the above allegations. LPA met with Administrator Ruth Fromme, and explained the purpose of the visit.

LPA conducted interviews with Ms. Fromme, and Resident One's (R1) daughter, and was able to confirm that R1 lives at the independant living side of The Palms at La Quinta that does not hold a license for care of its residents due to the residents being independant.

Thus, LPA determined that the complaint was UNFOUNDED, meaning that the allegations were false, could not have happened and/or is without a reasonable basis.

An exit interview was conducted with Ms. Fromme and a copy of this report was provided.
