Facility Evaluation Report

Licensing Program Analyst (LPA) Mary Rico conducted an unannounced case management visit. LPA Rico met with Administrator Dulce Redford and explained the purpose of the visit. During today’s visit, LPA Rico conducted staff interviews, reviewed facility’s documents, and toured the facility.

Upon record review, it was revealed that R1 was admitted to the facility on 11/21/2025. During record review, LPA observed that R1 was sent to the hospital on 12/10/2025 due to medical condition, as of 12/19/2025 R1 still remains at hospital. During staff interviews, the Administrator stated R1 still resides at the facility and is not in the process of evicting R1. LPA Rico toured the facility and observed R1 personal belongings remain at the facility.

Based on the information gathered on today’s visit no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and this report (LIC809) was discussed and provided to Administrator Dulce Redford.
