Facility Evaluation Report

Licensing Program Analyst (LPA), Seo Jeon, conducted an unannounced visit to the facility for a case management to follow up on an incident report received by the Department on 09-27-2022. The LPA was allowed entrance into the facility and met with Blanca Santamaria, Program Administrator. The LPA informed the Program Administrator of the purpose for the visit.

The LPA conducted a tour of the interior and exterior areas of the facility, conducted a review of records, obtained, and requested copies of pertinent documentation. LPA's review of resident record revealed Resident #1 (R1) has not resided at the facility since October 2022. LPA conducted interviews with residents and staff. LPA did not observe any immediate health and safety concerns.

There are no deficiencies being cited, per California Health & Safety Code and Code of Regulations, Title 22.

An exit interview was conducted, and a copy of this report was provided to the Program Administrator.
