Facility Evaluation Report

Licensing Program Analyst (LPA), Seo Jeon, conducted an unannounced visit to the facility for a case management to follow up on an incident report received by the Department on 08-08-2025 regarding Resident #1(R1), Staff #1 (S1), Staff #2 (S2) and Staff #3 (S3). The LPA was allowed entrance into the facility and met with Blanca Santamaria, Program Administrator. The LPA informed the Program Administrator of the purpose for the visit.

The LPA conducted a tour of the interior/exterior areas of the facility, conducted a review of records, obtained, and requested copies of pertinent documentation. LPA interviewed R1 and staff. LPA confirmed that both S1 and S2 were not present in the facility. LPA also confirmed that both S1 and S2 were not scheduled to be in the facility. LPA did not observe any health and safety concerns.

There are no deficiencies being cited, per California Health & Safety Code and Code of Regulations, Title 22.

An exit interview was conducted, and a copy of this report was provided.
