Facility Evaluation Report

Licensing Program Analyst (LPA), Armando Perez conducted a visit to the facility for the purpose of a case management. LPA met with Assistant Administrator O'Koia Nixon and informed her of the purpose of the visit.

On June 6, 2025, Community Care Licensing received an incident report indicating staff member (S1) engaged in a verbal altercation resulting in a closed fist punch to client’s (C1) face. During the site visit, the LPA conducted a facility tour and found no health or safety concerns. The LPA also interviewed staff and clients and obtained relevant documentation regarding the incident.

The Assistant Administrator informed LPA that S1 was immediately suspended on the same day of the incident on June 5, 2025, upon notification of the incident. Furthermore, based on additional information obtained by facility representatives, S1 was terminated effective immediately on June 10, 2025.

The LPA advised the Assistant Administrator that further investigation is required at this time.

An exit interview was conducted, and a copy of this report was provided to Assistant Administrator O'Koia Nixion
