Facility Evaluation Report

On 12/29/25 Licensing Program Analyst (LPA) Javina George made an announced visit to the facility to conduct a closure visit. LPA met with Dijon Washington, CEO. The Licensee has decided to surrender the license as there have been hurdles with completing the venodrization process.

During today's visit LPA conducted a walk thru of the facility and verified that there are no clients in care. LPA did not observe any indication that there are clients in care such as clothes, food, furniture. The home was licensed 02/03/25, and did not ever have clients placed at home. There was no physical license surrendered as the licensed was issued electronically.

An exit interview was conducted where a copy of this report was reviewed and provided to Dijon Washington, CEO.
