The inspector’s account

On 05/29/2026 Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced Case Management incident visit. LPA met with Facility Administrator Jordan Rush and explained the reason for today’s visit. The visit is pertaining to incident that occurred 04/30/2026 pertaining to Client #1 (C1).

During visit, LPA observed no clients at the facility. LPA spoke with Administrator and received pertinent information in regards to incident. LPA will follow up at a different time to speak with C1.

During today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations regarding this incident. An exit interview was conducted and this report was provided to Administrator Jordan Rush.
