On 02/11/2026 Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced visit for the purpose of health and safety checks. LPA met with Administrator Jordan Rush and explained the purpose of today's visit.
During today's visit, LPA conducted observations by touring the facility and obtained facility records.
No deficiency was cited during this visit. No imminent health or safety issues were observed. An exit interview was conducted where this report LIC809 was discussed and provided to Administrator Jordan Rush.
