Facility Evaluation Report
Licensing Program Analysts (LPA) Edith Conchas and Renese Howell-Small conducted an announced visit to the facility for the purpose of increase in capacity. LPA was greeted and granted entrance by Licensee David Corpin.
Per the LIC200, Licensee requested for the capacity increase from two (2) non-ambulatory and two (2) bedridden to four (4) non-ambulatory and or four (4) bedridden residents. The fire clearance request was approved on on 5/29/2025 four (4) non-ambulatory, for a total of four (4) bedridden clients.
The facility is a four (4) bedroom three (3) bath home.
The licensee was advised that the noted designated capacity for each room is to remain in compliance.
LPA observed that the client bedrooms were appropriately furnished and had functional lighting. The physical plant is ready for an change in capacity. LPA will update the facility's file and issue a new license stating change in capacity.
Licensee will be bringing in beds in two of the rooms and adding non-slip mats into the showers.
An exit interview was conducted where this report LIC809 was discussed and provided to the Licensee David Corpin .
