Facility Evaluation Report

Licensing Program Analysts (LPAs) Magda Malcore and Sarina Ramirez initiated a case management visit. LPAs discussed the purpose for the case management with Administrator Dorance Creihton.

On 4/22/25, LPAs arrived at the facility on a complaint investigation. LPAs were greeted and granted entry into the facility by staff #1(S1). LPAs determined through background clearance provider (Guardian) that S1 did not have a criminal record clearance and/or exemption through the Department. S1 stated that they work for another facility and they were at the facility until another staff arrives.

A deficiency is being cited and civil penalties are being accessed in accordance with Title 22, Division 6 of the California Code of Regulations.

An exit interview was conducted where this report was discussed. A copy of this report was provided with appeal rights to the Administrator at the conclusion of the visit.
