STATE OF CALIFORNIA - HEALTH AND HUMAN SERVICES AGENCY
This is an official report of an unannounced visit/investigation of a complaint received in our office on
07/10/2025 and conducted by Evaluator Mary Garza
COMPLAINT CONTROL NUMBER: 24-AS-20250710102232
Staff do not prevent a resident from sexually abusing another resident
On 10/29/25 Licensing Program Analyst (LPA) M. Garza arrived at the facility unannounced to complete a complaint visit. This visit is being conducted to deliver findings on complaint #24-AS-20250710102232. LPA met with Administrator, Pablo Armigo explained reason for visit and was permitted entry into the facility. No residents were present during today’s visit.
During the investigation interviews were conducted, documentation was gathered and reviewed. Review of police reports showed that R1 would invite R2 into their room to engage in communication and interaction with one another. Although it was discouraged for the R1 and R2 to spend time in such close quarters, staff was unable to violate R1’s personal rights and allow them to engage with one another. Interviews with staff disclosed R1 would go along with “sitting on R2s lap” and rough housing around however, when R1 would have enough R2 would not stop when asked. Although the allegation may or may not have occurred the allegation did not meet the preponderance of evidence standard of the California Code of Regulations, Title 22. This allegation is UNSUBSTANTIATED. No deficiencies cited during todays visit.
Exit interview completed with Administrator, Pablo. A copy of this report was provided.
