On 5/12/23 LPA attempted visit with Administrator Jose. Administrator was contacted and stated there was no one at the facility and they were unavailable to come to the facility at this time. Designee was off for the day. Administrator was informed per LPM the visit will be completed another time but they need to have someone available within a reasonable time. LPA will return to complete visit.
