Facility Evaluation Report
On 4/18/2025 Licensing Program Analyst (LPA) M Vega arrived unannounced for an Annual Required Inspection. LPA met at the door with Staff. LPA stated the purpose of the visit and was allowed entry. During this visit LPA toured facility with Staff inside and out. Facility has one main entry and other entrance/exit points appropriately marked. At the time of the visit there were no clients present.
The facility is clean, with comfortable temperature recorded at 68 degrees Fahrenheit with no obstruction or fire clearance issues. Hand sanitizer was readily available to residents and visitors. Bathrooms have trash cans with lids. The exterior tour was conducted. Facility as five-bedroom house. Every room is fully furnished.
Kitchen observed with free of clutter and obstruction. Sharp items placed away and locked. The kitchen refrigerator was checked and 2- day supply of fresh perishables to be properly stored. LPA observed the required 7-day supply of non-perishable food.
Cleaning supplies were observed to be locked in a laundry room. Records were reviewed for infection control training.
Medications were observed to be locked. Facility uses electronic document file for administration and dispensing of medication. First aid kit was observed and contained all required items.
Carbon monoxide and smoke detectors were tested and observed to be operational.
A copy of the report was provided to staff and permission granted by administrator to sign report.
