Facility Evaluation Report

On 10/06/2025, Licensing Program Analyst (LPA) Daiquiri Boyd made an unannounced follow-up visit to the facility for the purpose of continuing the Annual Inspection visit.

LPA asked that Administrator be called to meet with me at the facility. Caregiver stated that Administrator Victor Flauta was at work at his office of medical practice. Caregiver said that she called Administrator Jill Stowell who oversees another of their facilities, and that she was in Bakersfield and couldn't meet me at the facility. LPA asked if there was an LIC308, designating someone to be able to assist when an Administrator is present. Caregiver said she asked Jill Stowell if there was someone designated and was told there was not.

LPA reviewed missing documents from residents files and reviewed the staff files. LPA found missing documents in staff files.

LPA found issues with maintenance in the backyard area and inside the home in the bathroom area.

LPA found that the Administrator they have listed as a backup, is not associated to this facility.

LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Listed documents shall be sent to Licensing.
