Facility Evaluation Report
Licensing Program Analyst (LPA) Daiquiri Boyd made an unannounced visit to the facility to conduct a Case Management visit to discuss a recently submitted Incident Report for incident occurring on 04/08/2026. House Manager Nancy Wade was at the home and met with LPA regarding this matter.
LPA toured the facility and checked that the exits had an auditory alarm or alerts when opened, front door alarm was active.
LPA reviewed relevant resident file and staff files.
It was reported that two staff were on duty at the time of this incident, as well as another staff that had come over to the home during the time of this incident.
It was explained to LPA that during the day of the incident, there were several visitors and the auditory door chime had been turned off, enabling a resident to walk out of the front door of the home unnoticed.
Citation was issued on this day. Signature of House Manager on this document acknowledges it's receipt.
