Facility Evaluation Report
Licensing Program Analysts (LPAs) Daiquiri Boyd and Jimmy Duarte conducted an unannounced annual visit to the facility and met with Manager(MA), Maria “Carmen” Muros. Licensee/Administrator (AD) Judit Jaime arrived shortly thereafter. LPA stated the purpose of the visit and was accompanied by MA while conducting the inspection of the facility. At the time of the visit, LPA observed 3 residents in the facility.
LPA toured the home and found three bedrooms and two bathrooms. Two bedrooms were single occupancy and one room has double occupancy. The hallway contained night lights for the clients use at night. Each room contained clean beds and linens. There were lights and ceiling fans operable. Screens were intact. One room had an exit that leads to the backyard, which was operable.
Hallway bathroom was clean and operable, grab bars, non-skid mat, shower seat, and covered trash can were observed. Paper towels were readily available. Water temperature measured at 108.2 F.
Hall closet contained linens and towels, as well as personal hygiene items for clients. Smoke alarm and carbon monoxide detectors observed and checked to be operating. Home temperature was set at 71 degrees.
Locked garage contained locked cabinets with chemical storage. Items stored in garage area were not of a hazard. Facility has a lawn service. Boards stored in the garage were moved in a way to not obstruct the exit to the backyard. Trash cans stored in the backyard area had lids. Gate from backyard to the front was self latching. Area was clean with sitting area for clients and staff. Handrails in backyard with ramp, were in good condition and not loose when shaken. Washer and dryer are kept in the garage and are in good condition.
