Facility Evaluation Report
On 8/14/25, Licensing Program Analyst’s (LPA) J. Leffall arrived unannounced to conduct an annual visit.
LPA introduced self, stated the purpose of the visit and was greeted by Staff (S1) Virginia Indrinal. Licensee Swellen Medroso Licensee (L1) arrived shortly after arrival. (L1) toured facility with both LPA. One client was present during inspection. The other client arrived later during visit from day program.
The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway
obstructions or fire hazards were observed inside or outside. An adequate supply of perishable and
non-perishable food was observed. Fire extinguisher was observed with a purchase date of 7/8/25. Fire drill
last completed in December of 2024. The temperature of the refrigerator was maintained at 38 degrees F and freezer at 0 degrees F. Medications observed locked in kitchen shelf. MARs were reviewed. Cleaning supplies and chemicals stored and locked under kitchen sink.
All bedrooms were observed to have the required furnishings and with adequate lightening. The bathrooms were toured. Bathrooms were observed operational during inspection. Hot water temperature was tested at a range of 105.6 and 108.5 degrees F in 2 bathrooms. Outside of facility toured and observed to be free of debris. Side gate observed self-closing and self-latching. Swimming pool observed locked and inaccessible to clients. Adequate outdoor seatings available for clients. Carbon monoxide and smoke detectors were tested and observed to be operational. All clients’ files were reviewed to have all the required documents. Staff files were also reviewed to have current First Aid/CPR, Personnel Record, Criminal record Statement, and Health Screening. Staff were fingerprinted cleared and associated with facility.
The following deficiency is being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6.
Exit Interview conducted. The following documents are requested and submitted to Fresno CCL by: 8/28/25. Forms requested: Lic 308, Lic 309 (if applicable), Lic 500, Lic 610D, Lic 9282, current Administrator certificate, and control of property. A copy of this report and appeal rights was provided to Licensee, whose signature on this form confirms receipt of this report.
