Facility Evaluation Report

On 08/04/26, Licensing Program Analyst (LPA) M. Yang conducted an unannounced case management visit regarding incident that had occurred. LPA met with staff Rosanne Salazar. Administrator Itaska Mask was called and unable to attend meeting.

The purpose of today’s visit is to follow up on incident that was reported by the Central Valley Regional Center where R1’s medication administration record (MAR) was observed not signed for dates 07/02/26 to 07/09/26. During the visit, R1’s medications were checked, MARs and Centrally Stored Medication List were reviewed, and records were reviewed.

Exit Interview conducted. A copy of this report was provided to staff, whose signature confirms receipt of this report.
