Facility Evaluation Report
On 3/16/26 Licensing Program Analyst, M. Garza arrived at the facility for an unannounced case management visit. LPA met with Direct Support Professionals, Boob Vongthalangsy and Chantha Moeuth. Administrator, Rosalind Ward was contacted. LPA explained reason for visit. LPA was informed it was okay to complete visit with staff. A health and safety check was completed on residents in care. 2 residents present during time of visit observed in common areas.
During visit LPA reviewed staff schedules, and completed interviews with Administrator via telephone and staff present at facility. LPA was informed S1 never worked at the facility. Review of staff schedules did not show S1 scheduled to work at the facility.
