Facility Evaluation Report
On 09/19/2025 Licensing Program Analyst arrived at the facility to complete an unannounced annual visit. LPA met with Direct Support Professional, Thavy Proum, explained reason for visit and was permitted entry into the facility. Administrator, Rosalind Ward was contacted and arrived a some time later. LPA completed a tour of the facility inside and out. A health and safety check was completed on residents in care. 2 residents present during visit.
Pathways and doors were clear and free from obstruction. Facility was without odor. Common areas were adequately furnished, and adequately lit. Smoke detectors and carbon monoxide detectors were present and operational at time of visit. Fire extinguisher last serviced on 2/25/25. Last fire drill on conducted on 9/13/25. Resident rooms observed to have the required furnishings and with adequate lighting. Sharps, chemicals and medications were located in locked cabinets/closets and cupboards. LPA observed sufficient seating under covered patio areas.
The following issues were observed during today’s visit: facility is fire cleared for 4 non-ambulatory residents, 1 of 4 residents is bedridden. Hallway cabinet missing knob in need of replacement. Fan in bedroom #2 observed dirty and in need of cleaning. Patio concrete observed with moss in need of removal. Emergency closet doors broken and in need of replacement/repair. Garage wall near water heater observed with 2 cut out holes (6 inches x 1 1/2 foot) in need of repair. Shower in bathroom #2 observed with molding in need of repair/replacement. Facility lacking details on disaster preparedness plan. Deficiencies cited per California Code of Regulations, Title 22 on the attached 809D. If not corrected, the violation with have a direct risk to the health, safety and/or personal rights of residents in care. CONT...
