On 7/1/26, Licensing Program Analyst (LPA) J. Leffall conducted a case management visit to follow up and confirm details of a report that was received by the Department. An incident was reported to CCLD and LPA visited the facility for follow up. LPA was greeted by Staff (S1) Micah Lermen. LPA introduced self and stated the purpose of the visit. LPA interviewed Licensee, Administrator, Staff and Resident.
At this time this case management needs further investigation.
