Facility Evaluation Report

On 5/12/2026 at 4:10pm Licensing Program Analyst (LPA) L. Hall arrived announced to conduct a Case Management visit. LPA met with Roche Castro, Administrator.

While LPA L. Hall was conducting a pre-licensing inspection LPA observed the following deficiency:

LPA was informed the Administrator has been on a leave of absence for 5 (five) weeks.

During record review LPA observed CCLD was notified of that R1 was placed on hospice.

The deficiencies were observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
