On 7/28/2022 at 3:30PM , Licensing Program Analyst (LPA) L. Ibo arrived unannounced to conduct a case management visit. LPA met with Executive Director Soledad Martinez, and explained the reason for the visit.
LPA went to the facility to deliver an immediate exclusion letter and verified that S1 was no longer working at the facility. LPA delivered letter to Soledad Martinez. LPA confirmed that S1 was not at the facility.
No deficiencies are being cited on this date.
