Facility Evaluation Report
On 09/25/2025 at 9:40 AM, Licensing Program Analysts (LPAs) P.Manalo and K.Nguyen arrived unannounced to conduct a Case Management visit. LPAs met with Direct Care Staff, Doreen Camara, and explained the purpose of the visit. Administrator gave verbal authorization for staff to sign the report.
During today's visit, LPAs confirmed with Administrator, Tylerjames Marcelo, regarding the Default Decision and Order for Staff 1 (S1) and if the order has been received. It is confirmed by Administrator, Staff 2 (S2), Staff 3 (S3), that S1 has not been at the facility nor seen at the facility. LPAs confirmed with Administrator that S1 is disassociated from the facility via proof of email.
