On 08/27/2025 at 1:35 PM, Licensing Program Analysts (LPAs) P.Manalo and L. Fontanilla arrived unannounced to conduct a Case Management visit.
While at the facility, LPAs observed Client 1 (C1) ambulating with one staff and bumping into the wall. LPAs noticed C1 walking unsteadily with the eyes closed.
LPAs spoke with Licensee, Arleen Marcelo, that the facility will send a written noticed to the fire department within 24 hours. Licensee will also send the LIC200, updated facility sketch, and request for non-ambulatory status for the rooms downstairs by 09/02/2025.
