LPA Luisa Fontanilla conducted a case management-incident visit as a follow up on the self-reported incident when client 1 (C1) left the facility without staff knowledge using the window. LPA met with Administrator Tyler James Marcelo.
During the visit, LPA obtained C1's Physician's Report, IPP and ISP.
