Facility Evaluation Report
On 11/24/2025 at 10:20 AM, Licensing Program Analysts (LPAs) P. Manalo and L.Fontanilla conducted a Health & Safety inspection as a result of Complain t #15-AS-20251121135610 . LPA met with Administrator, Betchie Terrado, and explained the purpose of the visit.
LPA toured facility including but not limited to the bedrooms, bathrooms, common area, kitchen, and outdoor area. Hot water temperature was measured at 110 degrees Fahrenheit. 7-day of non-perishable and 2-day of perishable food supplies were sufficient. Smoke detectors and carbon monoxide are in operating condition. First-aid kit was complete. Fire extinguisher was observed to be full and last serviced on 06/26/2025. There are no accessible bodies of water observed. Indoor and outdoor passageways are free of obstruction.
The following deficiencies were observed:
At 10:02 AM, LPAs observed the emergency side gate latch wrapped around with tie.
At 10:03 AM, LPAs observed multiple boxes in the backyard outside the trash bin.
At 10:11 AM, LPAs observed Lysol wipes and Lysol Disinfectant Spray in all the clients' room and clients' bathroom unlocked.
At 10:14 AM, LPAs observed an unlocked tupperware labeled as Hydrocortisone Cream 1% in client's room. Staff interviewed stating it was C1's medications.
Deficiencies were cited per Title 22 California Code of Regulations (see Lic 809D).
Exit interview was conducted with Terrado. Copy of this report, Appeal Rights, and LIC421 was provided.
