Facility Evaluation Report
On 1/7/2026 at 10:30 am, Licensing Program Analyst (LPA) Y.Brown conducted an unannounced annual required inspection. LPA met with Program Dire ctor Dominique Mellion
The facility’s fire clearance was approved for forty-six (46) clients, ten (10) may be non-ambulatory. LPA toured the facility including but not limited to activity rooms, bathrooms, kitchen, office spaces, common areas and the outside recreational area. Clients bring their own lunches and snacks but the programs has extra food and snacks at the facility. Medications are not handled/dispensed by this program. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ restroom was measured at 107.4 degrees Fahrenheit.
All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for clients. Smoke and carbon monoxide combination were observed during visit. Fire extinguisher last serviced on 2/3/2025. The Fire Drill was reviewed to be last conducted on 12/22/2025.
