Facility Evaluation Report
On 5/20/2026 at 9:00 AM, Licensing Program Analyst (LPA) Y. Brown arrived unannounced to do an annual 1-year required inspection. LPA met with care staff Clarisse Taklie and Pam ela Temple and explained the purpose of the visit.
LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area and garage. All indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 73 degrees Fahrenheit. The water temperature in the shared clients bathroom was measured at 119.1 degrees Fahrenheit. The Emergency Disaster Plan was last reviewed on 4/10/2026. Emergency disaster drills were last conducted on 5/10/2026. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients.
There is a minimum of one-week supply of non-perishables and 2-day perishables food supply. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for clients. Smoke detectors and carbon monoxide combination were in operating condition during visit. Fire extinguisher was last serviced on 2/5/2026. First aid kit was observed to be complete. LPA reviewed four (4) client records and three (3) staff records and all were current and complete. LPA reviewed a sample of medication. LPA reviewed three (3) client P & I.
