Facility Evaluation Report

While at the facility investigating a complaint (Control # 15-AS-20240311152642), Licensing Program Analyst (LPA) Delmundo observed the laundry room where laundry supplies are kept was unlocked. LPA informed staff, Gendelle Nebril Camarillo who was on the floor at the time. LPA also informed Lisa Lostica.

Deficiency is cited from Title 22 California Code of Regulations, and listed on 809D. Failure to submit proof of correction by plan of correction due date, and any repeat violation within 12 month period may result in civil penalty.

Deficiency and plan and proof of correction were discussed with the Jeralyn May.
