Allegation : Staff do not keep the facility free of pests or rodents
On 12/17/2025 and 1/8/2026, LPA inspected facility and grounds with S1. LPA checked all residents’ bedrooms, kitchen, bathrooms, living room, dining room, hallways, laundry room and did not observe any rodent feces/droppings inside the facility. Interview with staff revealed that the Administrator has put out rat traps in the facility during winter or summer months due to the weather for caution but have not witnessed any rodents. LPA observed no rodents present during visit.
Allegation : Staff do not provide adequate food service to residents
Interviews with clients revealed that the staff are open to making meals upon request, if there's something on the menu the residents doesn't want. Interview with clients revealed that the facility provides enough food for the clients to eat. LPA observed there were a minimum of 7 day of nonperishable and 2 days of perishable foods available for clients in care.
Allegation: Staff does not keep the cleaning products or disinfectants locked up away from residents
On 12/17/2025 and 1/8/2026, LPA inspected the facility with S1. LPA checked all cabinets and observed that all of the cabinets and drawers that had disinfectants or cleaning products were locked and inaccessible to clients.
Allegation: Staff are violating the personal rights of clients.
