Facility Evaluation Report

On 8/1/2025 at 10:00 am, Licensing Program Analyst (LPA) Y. Brown conducted a Case Management visit regarding closure of facility. LPA met with Olga Pinto, Administrator, and explained the purpose of the visit.

LPA toured facility including but not limited to bedrooms, bathrooms, kitchen, and outdoor areas. LPA observed that no clients were present during inspection. Administrator stated that they did not have their physical license but the LPA obtained an email regarding the notice of the facility closure and forfeiting their license.

Administrator stated the last client was discharged on July 30, 2025, at 12:00 pm, and that the facility has been closed.

LPA will send a forfeiture letter to the licensee at a later time.
