Facility Evaluation Report
On 1/21/2026 at 9:30 AM, Licensing Program Analyst (LPA) Y. Brown arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator Adeniyi and explained the purpose of the visit. The facility’s fire clearance was approved for fourteen (14) all ambulatory only. The administrator currently holds a certificate (# 6079471735 ) that expires on 10/21/2027.
LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 7 total bedrooms which all bedrooms are occupied by the clients and two bathrooms. There are no bodies of water. A comfortable temperature for clients is maintained at 72 degree Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. All toilets, hand washing and bathing are safe, sanitary and in operating condition. The supply of hygiene products was available for clients.
There is a minimum of one week supply of nonperishable and 2-day perishables food supply. The water temperature at the facility was measured at 111.2 degrees Fahrenheit. Smoke detectors and carbon monoxide were in operating condition during visit. LPA reviewed four (4) staff and six (6) client files. LPA observed a sample of client medication.
