The inspector’s account

On 01/14/2025 at 8:50 AM, Licensing Program Analysts (LPAs) P. Manalo and L. Fontanilla arrived unannounced to conduct 1-Year Annual Required inspection. LPAs met with Staff, Sara Haile, who phoned the Administrator and explained the purpose of the visit. The backup Administrator, Frank Starks, came shortly after. The facility’s fire clearance was approved for fourteen (14) all ambulatory only.

LPAs toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 7 total bedrooms which all bedrooms are occupied by the clients and two bathrooms. There are no bodies of water. A comfortable temperature for clients is maintained at 72 degree Fahrenheit. LPAs observed lighting in all rooms are adequate for the comfort and safety of the clients. All toilets, hand washing and bathing are safe, sanitary and in operating condition. The supply of hygiene products was available for clients. There is a minimum of one week supply of nonperishable and 2-day perishables food supply.

Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 07/17/2024.

At 9:05 AM, 7 of clients records were reviewed. At 9:25 AM, 3 staff records were reviewed and all 3 are associated to the facility. LPAs reviewed four samples of clients' medications.
