Facility Evaluation Report

On 6/3/2026 at 11:40 am, Licensing Program Analyst (LPA) Y. Brown conducted an unannounced annual required inspection. LPA met with Samantha Atendido, Facility Manager and explained the purpose of the visit.

The administrator currently holds a certificate (# 7033550735) that expires on 4/23/2027 . LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. All indoor and outdoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients.

Hot water temperature in the shared clients’ bathroom was measured at 114.8 degrees F. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for clients. There is a minimum of 7-day non-perishables and 2-day perishables foods. Smoke detectors and carbon monoxide were observed during visit. Fire extinguisher was last serviced on 03/30/2026. Emergency disaster plan last reviewed on 3/25/2026. Fire drill last conducted 5/2/2026. First aid kit was observed to be complete. LPA reviewed three (3) clients and five (5) staff records. LPA reviewed a sample of medication.
