Facility Evaluation Report

On 9/22/2025 at 3:30PM, Licensing Program Analysts (LPAs) G. Luk and Y. Brown arrived unannounced to conduct a case management visit. LPAs met with Administrator, Wardell Jackson.

During the course of investigation for complaint (#15-AS-20250917084942), the following deficiencies were observed.

1) LPAs reviewed Guardian and observed S3 and S4 are not fingerprint cleared. Civil penalty of $1,000 is being assessed.

2) LPAs observed food was stored with cleaning supplies in the storage shed.

The deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiencies may result in civil penalties.

Exit interview conducted. A copy of this report, civil penalty, and appeal rights provided.
