Facility Evaluation Report

On 11/18/2025 at 9:45 AM, Licensing Program Analyst (LPA) Y. Brown conducted an unannounced annual 1-year required annual inspection. LPA met with care giver Rhyle Castro and explained the purpose of the visit. Rhyle phoned Administrator Jacqueline Adams and she arrived at around 10:45 AM.

The administrator currently holds a certificate (#7025858740) that expires on 8/24/2027. The facility’s fire clearance was approved for six (6) residents, five (5) non-ambulatory and one (1) bedridden residents. LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. All indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 71.5 degrees Fahrenheit. LPAs observed lighting in all rooms are adequate for the comfort and safety of the clients.

Hot water temperature in the shared residents bathroom was measured at 114.5 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for residents. Smoke detectors and carbon monoxide combination were in operating condition during visit. Fire extinguisher was last purchased on 3/14/2025. First aid kit was observed to be complete.
