Facility Evaluation Report

On 3/3/2026 at 10:30 AM, Licensing Program Ana lyst (LPA) Y. Brown arrived unannounced to do an annual required inspection. LPA met with care staff Sheila Bengco and explained the purpose of the visit. Sheila phoned the administrator (ADM), Gabriele Sibonga and ADM arrived to the facility shortly after.

The administrator currently holds a certificate (# 7016463735 ) that expires on 6/26/2026. LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, and back yard. All indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 73 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for residents. Smoke detectors and carbon monoxide combination were in operating condition during visit. The water temperature in the shared clients restroom was measured at 108.8 degrees Fahrenheit. Fire extinguisher was last purchased on 10/1/2025. First aid kit was observed to be complete. LPA reviewed five (5) client records and five (5) staff records. LPA reviewed a sample of medication. LPA reviewed 5 client's P & I.
